    How can I encourage multi-stakeholder narrative and reflection on the use of ICT in Teacher Professional Development programmes in Rwanda?

    This is an action research enquiry into how I can improve my practice to encourage multi-stakeholder narrative and reflection on the use of Information and Communication Technology (ICT) in Teacher Professional Development (TPD) programmes in Rwanda. I examine the complexity of the ICT-TPD landscape in the Africa Region. I describe two action research cycles in which I attempt to encourage reflection on ICT in professional development in Rwanda. In each cycle I explore the potential of an Activity Theory lens for probing the issues and examining the perspectives of the stakeholder community of teachers, teacher educators, curriculum developers and researchers affiliated to national ICT in TPD programmes and initiatives. I integrate a ā€œMost Significant Changeā€ narrative technique to engage participants in telling stories of significant change in their practice with technology integration. Through the rigour of the action research living theory approach I come to a number of conclusions about my own values and how I actually live my values in practice as I engage with partners in discourse and reflection for mutual learning on the issues of ICT integration in Teacher Professional Development

    Algorithm Development and Validation for Satellite-Derived Distributions of DOC and CDOM in the US Middle Atlantic Bight

    In coastal ocean waters, distributions of dissolved organic carbon (DOC) and chromophoric dissolved organic matter (CDOM) vary seasonally and interannually due to multiple source inputs and removal processes. We conducted several oceanographic cruises within the continental margin of the U.S. Middle Atlantic Bight (MAB) to collect field measurements in order to develop algorithms to retrieve CDOM and DOC from NASA's MODIS-Aqua and SeaWiFS satellite sensors. In order to develop empirical algorithms for CDOM and DOC, we correlated the CDOM absorption coefficient (a(sub cdom)) with in situ radiometry (remote sensing reflectance, Rrs, band ratios) and then correlated DOC to Rrs band ratios through the CDOM to DOC relationships. Our validation analyses demonstrate successful retrieval of DOC and CDOM from coastal ocean waters using the MODIS-Aqua and SeaWiFS satellite sensors with mean absolute percent differences from field measurements of < 9 %for DOC, 20% for a(sub cdom)(355)1,6 % for a(sub cdom)(443), and 12% for the CDOM spectral slope. To our knowledge, the algorithms presented here represent the first validated algorithms for satellite retrieval of a(sub cdom) DOC, and CDOM spectral slope in the coastal ocean. The satellite-derived DOC and a(sub cdom) products demonstrate the seasonal net ecosystem production of DOC and photooxidation of CDOM from spring to fall. With accurate satellite retrievals of CDOM and DOC, we will be able to apply satellite observations to investigate interannual and decadal-scale variability in surface CDOM and DOC within continental margins and monitor impacts of climate change and anthropogenic activities on coastal ecosystems

    Remote Sensing of the Absorption Coefficients and Chlorophyll a Concentration in the U.S. Southern Middle Atlantic Bight from SeaWiFS and MODIS-Aqua

    At present, satellite remote sensing of coastal water quality and constituent concentration is subject to large errors as compared to the capability of satellite sensors in oceanic waters. In this study, field measurements collected on a series of cruises within U.S. southern Middle Atlantic Bight (SMAB) were applied to improve retrievals of satellite ocean color products in order to examine the factors that regulate the bio-optical properties within the continental shelf waters of the SMAB. The first objective was to develop improvements in satellite retrievals of absorption coefficients of phytoplankton (a(sub ph)), colored dissolved organic matter (CDOM) (a(sub g)), non-pigmented particles (a(sub d)), and non-pigmented particles plus CDOM (a(sub dg)), and chlorophyll a concentration ([Chl_a]). Several algorithms were compared to derive constituent absorption coefficients from remote sensing reflectance (R(sub rs)) ratios. The validation match-ups showed that the mean absolute percent differences (MAPD) were typically less than 35%, although higher errors were found for a(sub d) retrievals. Seasonal and spatial variability of satellite-derived absorption coefficients and [Chl_a] was apparent and consistent with field data. CDOM is a major contributor to the bio-optical properties of the SMAB, accounting for 35-70% of total light absorption by particles plus CDOM at 443 nm, as compared to 30-45% for phytoplankton and 0-20% for non-pigmented particles. The overestimation of [Chl_a] from the operational satellite algorithms may be attributed to the strong CDOM absorption in this region. River discharge is important in controlling the bio-optical environment, but cannot explain all of the regional and seasonal variability of biogeochemical constituents in the SMAB

    Physician associates and GPs in primary care: a comparison.

    BACKGROUND: Physician associates [PAs] (also known as physician assistants) are new to the NHS and there is little evidence concerning their contribution in general practice. AIM: This study aimed to compare outcomes and costs of same-day requested consultations by PAs with those of GPs. DESIGN AND SETTING: An observational study of 2086 patient records presenting at same-day appointments in 12 general practices in England. METHOD: PA consultations were compared with those of GPs. Primary outcome was re-consultation within 14 days for the same or linked problem. Secondary outcomes were processes of care. RESULTS: There were no significant differences in the rates of re-consultation (rate ratio 1.24, 95% confidence interval [CI] = 0.86 to 1.79, P = 0.25). There were no differences in rates of diagnostic tests ordered (1.08, 95% CI = 0.89 to 1.30, P = 0.44), referrals (0.95, 95% CI = 0.63 to 1.43, P = 0.80), prescriptions issued (1.16, 95% CI = 0.87 to 1.53, P = 0.31), or patient satisfaction (1.00, 95% CI = 0.42 to 2.36, P = 0.99). Records of initial consultations of 79.2% (n = 145) of PAs and 48.3% (n = 99) of GPs were judged appropriate by independent GPs (P<0.001). The adjusted average PA consultation was 5.8 minutes longer than the GP consultation (95% CI = 2.46 to 7.1; P<0.001); cost per consultation was GBP Ā£6.22, (US$ 10.15) lower (95% CI = -7.61 to -2.46, P<0.001). CONCLUSION: The processes and outcomes of PA and GP consultations for same-day appointment patients are similar at a lower consultation cost. PAs offer a potentially acceptable and efficient addition to the general practice workforce

    Should I stay or should I go? Patient understandings of and responses to source-isolation practices

    Isolation of patients, who are colonised or infected with a multidrug-resistant organism (source-isolation), is a common practice in most acute health-care settings, to prevent transmission to other patients. Efforts to improve the efficacy of source-isolation in hospitals focus on healthcare staff compliance with isolation precautions. In this article we examine patientsā€™ awareness, understandings and observance of source-isolation practices and directives with a view to understanding better the roles patients play or could play in transmitting, or limiting transmission, of multidrug-resistant organisms (MRO). Seventeen source-isolated adult surgical patients and two relatives participated in video-reflexive ethnography and interviews. We learned that, although most of these patients wanted to protect themselves and others from colonisation/infection with a MRO, they had a limited understanding of what precautions they could take while in isolation and found it difficult to obtain ongoing information. Thus, many patients regularly left their source-isolation rooms without taking appropriate precautions and were potentially contributing to environmental contamination and transmission. Some patients also interacted with other patients and their personal belongings in ways that exposed other patients, unnecessarily, to colonisation/infection risk. By not providing patients with adequate information on infection risk or how they could contribute to their own safety or that of others, they are denied the opportunity to fully engage in their healthcare. To improve the efficacy of source-isolation and contact precautions in general, patient care providers should consider colonised or infected patients as active partners in reducing transmission and involve patients and relatives in regular, ongoing conversations about transmission prevention
